Information Sheets & Forms

There will be occasions when our administrator, Alberta Pensions Services Corporation (APS), will need information from you to keep your pension record current or to make a pension transaction on your behalf. For example, APS may need to verify your date of birth if you want to transfer your pension benefits from another plan or retire.

The forms needed for these situations are listed below, among others. For your convenience, we have included information sheets with most of the forms. Read the information carefully, then:

  • Print the form and complete it by hand.
  • Sign and date the form.
  • Send the form to APS at the address provided on the form.

To be valid, the paper copy of the form must be signed by hand, in ink. No form of electronic signature is acceptable for pension purposes.
